The City of Edmonds requires that you complete a Request for Public Records before release of public information. Please print and complete the form and mail to the Edmonds Fire Department, 121 Fifth Avenue North, Edmonds, WA 98020, or return-fax to 425-775-7721.

If you are requesting a Medical Incident Report, a medical release form signed by the patient or client must accompany the completed Request for Public Records.

We will respond to your request within five business days of the receipt of your signed Request for Public Records.

RCW 42.17.300 and 42.260(7) and (8) permit agencies such as cities to impose a reasonable charge for providing photocopies of public records. City of Edmonds Resolution No. 1154 allows for the provision of photocopies at the following rate: Incident Reports ~ $10 per report up to ten pages; 15-cents for each additional page.
